Submit a Work Request

Use this form to request service from the Facilities Operations Department including Maintenance, Housekeeping, Utilities and Energy (UES) and Landscaping and Natural Resources (LNR).

This page is for routine maintenance requests only. Please do not enter emergency requests through this form. An emergency request is a time sensitive situation such as inoperative elevator, toilets running over, power out, or an alarm that will not turn off. In a non-life threatening emergency, call the Work Control Center 24/7 anytime day or night. (407) 823-5223

If you are reporting a life threatening emergency, call 911 immediately! Examples are but not limited to: elevator entrapment, fire, traffic accidents, etc.

 

To submit a request, follow these steps:

1) Fill in your personal information, making sure to fill in the required fields denoted by an asterisk.

2) Give a description of the problem you are having. Please be as detailed as possible so we may better serve you.

3) Click the submit button on the bottom of this form. You'll receive an e-mail confirming your form has been submitted.
